I would ask about the car because obviously the spouse isn't hurt bad enough that they can't call. And also if the car isn't hit too bad then they couldn't have been hurt to bad either.

I guess the right answer would be to ask if they are hurt, but personally, I'd figure that if she was calling about the accident herself, then she must be okay, otherwise somebody else would be calling me.
I'd ask how the car had fared first because it would gage just how bad the accident was, and how much moolah I'd be paying out in the end, as well as allowing me to solve the question of how I'm going to work transpo out if the car is totaled or otherwise undriveable.
Whether it was her fault would be later determined by the responding officers and insurance companies. Nobody who gets into an accident should ever admit to being at fault to anyone, ever, just in case the question is asked of that person later.
Out of curiosity, why do you ask? You didn't get into an accident again, didja?
My brother Jason's wife did. He asked about the car first, which I see as answering how she's doing. She was mad at him, so I just wanted to see what others thought.
Oh, well, there's no way he's going to make that right with her. She'll be pissed off at him no matter what.
You're spouse could still call you if they are hurt. I still would ask if they are hurt. Cars can be replace, spouses can't.

You don't ASSume that the spouse is fine! The best way to gauge how bad the accident is, is to ask, "Are you okay? Is my child hurt?"
Not, "Just great! How bad is the car?! How much do I have to fork over for the damage?!"
Heaven forbid you show some signs of concern for your wife.
